Question for Ken about book publicity

I'm new around here so I may be asking questions that have long-ago been answered, but I was curious how book publicity tours work. When you appear on radio, or TV, or at a book signing do those places pay for your appearance there, for the business you bring in? Or does the publisher pay them for the advertising of the book? Are you reimbursed for your time or is it just written into your book contract that the money you're paid for writing also pays for X number of publicity appearances? Also, how much say do you get in what the final product of the book looks like? Obviously you wrote it, but what about cover design, typeface, etc. Is your input solicited for any of that?
